Submit a Claim

Your claims can be emailed, mailed or faxed to the Difference Card to receive reimbursement. Be sure to complete a Difference Card Reimbursement Form and submit it along with your Explanation of Benefits (EOB).

Your claim will be processed within 4-12 business days from the date the claim is received and you will receive a statement from the Difference Card with your determination and reimbursement check.

If you want to enroll in direct deposit and your employer allows this option, you can change your reimbursement method to "Direct Deposit" when you log into the Participant Portal.

REMEMBER: The most important document that the Difference Card needs to reimburse you for your deductible, coinsurance or copay is your Explanation of Benefits (EOB). You can always obtain a copy of your EOB through your Insurance Provider's website.
